This form is about your personal medical history, which includes your past and present treatments, symptoms, and concerns.
Attend a 60-minute first visit appointment
Your initial visit will include a full evaluation of your medical condition(s). Most of the time, at the first visit, medications are prescribed or refilled.
Have your psychiatrist determine your personalized plan and medication
At this phase, you and your provider will be agreeing on a personalized plan and treatment. Your prescriptions can be electronically sent to your pharmacy.
